Paper Surveys

Why do paper surveys?

• To accomodate parents who do not easily have access to a computer
• To include the survey in a mailing that goes home to parents. Some schools mail registration materials during the summer. The survey is included as a required document that needs to be returned at the beginning of the school year.

How does this process work?

• We design the standard online version of a survey.
• An additional version is created so that paper surveys can be read by the scanner.
• Clients are provided a pdf version of the survey so you can print it out.
• When surveys are returned to the school, put them in a box and mail them off to Online School Surveys for processing.
• Both online and paper surveys can be executed to collect data for a single survey. If some parents want to use the computer, that works. If other parents prefer to use paper and pencil, that is fine, too.
• Paper surveys are scanned at the Online School Surveys office. The data is deciphered by the survey scanning software. Data from the online surveys and the paper surveys are compiled together.

How much does it cost to do paper surveys?

It costs an additional $200 - $500 for paper surveys because more time and labor is involved.

$200: two page survey; includes printing by Online School Surveys, mailed UPS to your location; per 1000 surveys or 2000 pieces of paper

$300: three page survey; includes printing by Online School Surveys, mailed UPS to your location; per 1000 surveys or 3000 pieces of paper

$400: four page survey; includes printing by Online School Surveys, mailed UPS to your location; per 1000 surveys or 4000 pieces of paper

$500: five page survey; includes printing by Online School Surveys, mailed UPS to your location; per 1000 surveys or 5000 pieces of paper

The client is responsible for paying for shipment of surveys back to Online School Surveys. Making the paper version of the survey and scanning the responses is included in the fee.

Because of the way the software works, the sizing of the survey has to be exact. Making copies on a photocopy machine skews the sizing of the image. We can only process surveys that are printed on our printer to make sure the sizing is consistent.
